The University of Louisville Institute for Sustainable Health & Optimal Aging Geriatrics Workforce Enhancement Program (GWEP) is a project funded by the Human Resources and Services Administration (HRSA) to improve the health and well-being of older adults living in six counties in Kentucky: Barren, Bullitt, Hart, Henry, Metcalfe, and Shelby counties. GWEP seeks to improve the quality of care for rural older adults through the interdisciplinary patient-centered care coordination and the development of a health care workforce (ICCOA) that improves health outcomes for older adults. Through this model of care, the Flourish Care Model, we are integrating geriatrics with primary care, maximizing patient and family engagement, and transforming our rurual health care system.
